What is an Umbrella Hub Organization (UHO)?

 An Umbrella Hub Organization (UHO) serves as a hub for affiliated organizations to support the sustainable delivery of the National Diabetes Prevention Program (NDPP). The UHO supports affiliates by aggregating data for the Diabetes Prevention Recognition Program (DPRP), sharing infrastructure costs, providing administrative functions (e.g., billing and claims submission), and leveraging best practices. Beyond this pilot project, HPCA envisions expanding this UHO will to other evidence-based chronic disease prevention and management programs for our network of health centers.
